L'identikit del caregiver
“Io certe volte, mi confesso, devo dire la verità, mi fa anche rabbia dirlo, certe volte mi arrabbio, le dico “basta non fare così”. E lei mi abbraccia, pur avendo questa patologia, però io dico che forse non siamo preparati, secondo me ci vuole anche una preparazione per poter stare accanto a queste persone.”
Ciao! In questo articolo vorrei spiegarti chi è il caregiver, quali sono i 4 step per una cura "autentica" e infine dei "pensieri neri del caregiver".
Una parola che spesso sentiamo quando parliamo di anziani fragili o di demenze è il “caregiver”, chi è questa figura?
La parola “caregiver” deriva dall’inglese “care= curare” e “giver= fornire”. Purtroppo, non ha una traduzione italiana e in linea di massima possiamo tradurlo con “colui o colei che si prende cura di”. Nel nostro caso i caregiver sono tutte quelle persone che si occupano di un anziano, sia esso sano o con demenza.
Il caregiver quindi è la persona che si assume il principale carico di assistenza. Si tratta spesso di un ruolo silenzioso e nascosto che è però fondamentale per chi ha perso la propria indipendenza.
Esistono, quando si tratta di una persona con difficoltà, due tipologie di caregiver:
- I caregiver formali, cioè tutte quelle persone che si prendono cura di altri per lavoro. Sono quindi caregiver formali tutti i professionisti sanitari come il medico, l’infermiere, l’operatore socio-sanitario e tutte le persone che assistono la persona anziana dietro il pagamento di un compenso;
- I caregiver informali (definiti anche caregiver familiari) sono invece tutte le persone, generalmente i familiari, che assistono la persona gratuitamente. Il caregiver informale è quindi la persona che all’interno della famiglia si assume il ruolo di cura.
Vediamo qualche numero riguardo a come sono distribuiti i caregiver in Italia.
Da un’indagine di qualche anno fa (EHIS,2015) i caregiver nel nostro paese erano circa 7,3 milioni. Da un’indagine dell’Istat (2010), è emerso che questo ruolo è svolto principalmente dalle donne (circa il 74%), di questa circa il 31% hanno un’età inferiore ai 45 anni, il 38% ha un’età compresa tra i 46 e i 60 anni, il 18% tra i 61 e i 70 anni e il 13% oltre i 70 anni.
Una persona con Alzheimer vive in famiglia circa 3 o 4 anni prima della ricerca di aiuto, ma l’assistenza a casa dura più a lungo. Il 96% dei malati, specie allo stadio iniziale, è totalmente gestito da familiari. Il caregiver dedica alla cura da 69 a 100 ore settimanali, mentre riserva a sé un'ora al giorno.
Questi dati ci fanno capire come i caregiver familiari, svolgono un ruolo fondamentale nella cura delle persone con demenza.
Trovo molto importante focalizzare l’attenzione sull’impatto degli aspetti della cura e dell’assistenza su chi li riceve. Proprio per questo alcune studiose hanno elaborato una serie di fattori che definiscono la “cura autentica”. Questi fattori ci permettono di farti capire come la cura che fornisci sia da un lato una “buona cura” e dall’altro di valorizzare il tuo impegno.
I quattro elementi descritti da Tronto, Barnes e Sevenhuijsen sono:
- Attenzione: si tratta di comprendere i bisogni del tuo caro in maniera onesta e consapevole. L’intera azione del prendersi cura parte proprio da questo elemento. Questo permette infatti di riconoscere la persona come tale, non come un “contenitore di bisogni”. Se non si presta questo tipo di attenzione, tutti i compiti assistenziali diventano una pura mansione da svolgere.
- Responsabilità: si tratta di assumersi la responsabilità dell’azione di cura. Assumersi questo onere significa compiere un’azione autentica e non standardizzata, implica quindi di avere consapevolezza che si potranno fare degli errori, i quali potrebbero nascere proprio dalla natura non standardizzata dell’azione.
- Competenza: per mettere in atto un’assistenza buona e utile è necessario che chi la implementa sia competente. Non stiamo parlando di competenze che siano solo tecniche, ma anche umane. Questo significa che si dovrebbe assistere la persona tenendo ben presente la sua unicità e rispettandola.
- Disponibilità: questo fattore si riferisce invece alla persona che riceve le cure. Si parla infatti della disponibilità della persona di ricevere la cura. La persona di cui ti occupi è vulnerabile: è importante che tu riesca ad avvicinarti emotivamente a lei per sviluppare una cura unica e autentica.
Tenere presenti tutti e quattro questi aspetti, ti permetterà di conservare intatte l’integrità e la dignità della persona di cui ti prendi cura e di vivere in maniera più positiva il tuo impegno.
Ci rendiamo conto che le richieste che vengono fatte dai professionisti per il caregiver sono molte: ti si chiede di essere sempre amorevole, di prestare attenzione ai cambiamenti, di avere pazienza e comprensione ogni ora di ogni giorno (anche quando non dormi per 24 ore). Proprio per quanto ti viene richiesto, spesso capita che il caregiver pensi di non farcela e spesso compaiono nella mente dei pensieri e delle emozioni che ci annebbiano. Di seguito ti propongo alcuni esempi di “pensieri neri” dei caregiver:
- L’impressione che tutti gli sforzi siano di scarsa utilità;
- La consapevolezza di non potersi ammalare, perché anche una breve assenza avrebbe conseguenze drammatiche sul congiunto;
- Il sommarsi di compiti assistenziali agli impegni familiari e lavorativi;
- L’alternanza di disgusto, compassione, rabbia, tenerezza, imbarazzo, vergogna (vs se stessi e vs il vostro caro) e sensi di colpa;
- La sensazione che la situazione sia destinata a durare all’infinito.
Questi pensieri sono normali per chi vive una situazione come la tua: ti ricordo che nessuno di noi è pronto ad affrontare cosa significa badare ad una persona cara affetta da demenza.
